
Obtaining Medical Records​​
How do I obtain a copy of my medical records?

Your complete medical record from our office will be provided to you on a USB flash drive, in PDF format. Faxing medical records to your new physician is not possible-the files are simply too large. You may bring the flash drive to your new physician.
A medical record release form ("release") signed by the patient, their legal guardian or parent is required. You can come into the office to sign a release or you can download a release from this website and email it to us at staff@fairhopefamilymedicine.com. This form can be downloaded from the link on the homepage. Complete the form by including your full identifying contact information including name, address, date of birth, signature and signature date. (No medical record number is needed) Include a witness signature as well. If someone other than yourself is dropping off your release form, it should be complete, including signature and witness before it is presented to the office. Also if you will have someone other than yourself pick up your medical record, please indicate that person or persons information in the release to section of the form. We will be providing all records so place a mark by the “ALL RECORDS” option.

We will copy the records to a USB flash drive and inform patients when they are ready to pick up. There will be a seven dollar ($7.00) fee for each patient’s set of medical records.

Before July 18, you can pick up your medical records at our office if we call you to indicate they are ready for pick up. After July 18, we will mail the flash drive. If mailing is required, we will add that cost of mailing to the seven dollar ($7.00) copy fee. After we receive the medical record release form from you, we will call you to collect the fee before we copy the records to a flash drive.

If you require your medical records to be printed out on paper, the fee is one dollar ($1.00) per page for the first 25 pages and fifty cents per page ($0.50) for each page thereafter plus a five dollar ($5,00) search fee plus the cost of mailing if necessary (these fees are prescribed by Alabama statute). We will first calculate the cost to print your medical records and call you to collect the fee before printing the records.

We are expecting many requests for medical records and will do our best to get them copied quickly. Medical records will be copied on a first come first serve basis. Accordingly, even if you request your records before July 18, we may not have them ready in time for you to pick them up at the office. In that case you will be charged a mailing fee if necessary.

This is subject to change based on US Postal Service fee changes. This includes the fee for delivery signature required and will be added to the seven dollar ($7.00) records fee. All mailed records will be sent by USPS with addressee delivery signature required (ie the patient/parent/legal guardian has to sign to receive the package.) After we receive the medical record release form from you, we will call you to collect the fee before we copy the records to a USB flash drive.
