Your medical records are available in a digital format, to be sent to you or your designee via encrypted email.

Any records for which we do not have specific instructions will remain on file according to the document retention policies of the Alabama Board of Medical Examiners. Information regarding those records may be obtained by contacting the custodian of medical records at staff@fairhopefamilymedicine.com.
